Job Summary

We are seeking a dedicated and experienced Pharmacist to join our healthcare team. The ideal candidate will play a vital role in ensuring safe and effective medication management, providing expert guidance to patients and healthcare providers. The Pharmacist will be responsible for medication dispensing, patient counseling, and collaborating with medical staff to optimize treatment outcomes.

Responsibilities

Dispense prescription medications accurately and efficiently in accordance with physician orders and OCP requirements Provide expert consultation (in English/ Chinese) to patients regarding medication usage, potential side effects, and interactions Collaborate with healthcare professionals to develop and review patient medication plans Ensure compliance with all regulatory standards and hospital policies related to medication safety and storage Monitor patient responses to medications and adjust therapy as needed in consultation with medical staff Maintain detailed documentation of medication dispensing and patient interactions Stay current with new medications, medical terminology, and advances in pharmacy practice to enhance patient care Participate in interdisciplinary team meetings to support comprehensive patient treatment plans

Requirements

Valid pharmacist license in Ontario of practice Kroll experience preferred Strong knowledge of medical terminology, medication administration, and patient care protocols Experience working with diverse patient populations Excellent communication skills for effective patient counseling and interdisciplinary collaboration Ability to work accurately under pressure in fast-paced environments Commitment to maintaining confidentiality and adhering to healthcare regulations Prior experience in clinical pharmacy settings is highly desirable

Pay: $45.00-$50.00 per hour
