PERTH AND SMITHS FALLS DISTRICT HOSPITAL DEPARTMENT: PHARMACY POSITION: CLINICAL PHARMACIST - PERMANENT FULL TIME SUMMARY OF DUTIES: Responsible for the provision of Clinical Pharmacy services; resolving and preventing drug related problems through medication profile review, patient assessment, Pharmaceutical Care Plan Development and Implementation. Professional activities include provision of drug information to other health care providers including, physicians, nursing, pharmacy personnel and other allied health professionals. Participants on committees, both internally and externally, conducts Drug Utilization Evaluation projects and is involved in presentation of in services to professional staff. EDUCATIONAL REQUIREMENTS \xc2\xb7 Completion of Bachelor of Science in Pharmacy, Pharm D, or undergraduate degree recognized by the Pharmacy Examining Board of Canada (PEBC) \xc2\xb7 Licensed as a Registered Pharmacist with the Ontario College of Pharmacists Desirable Experience: \xc2\xb7 2 years previous hospital pharmacy experience or Hospital Pharmacy Residency Assets and Abilities: \xc2\xb7 Excellent communication, interpersonal and team building skills \xc2\xb7 Strong team skills with a proven ability to perform duties independently, with a high degree of motivation and minimal supervision \xc2\xb7 Strong knowledge of Pharmaceutical drugs, therapeutics with ability to apply during provision of practice \xc2\xb7 High degree of accuracy and attention to details \xc2\xb7 Demonstrated computer literacy \xc2\xb7 Innovative and willing to seek new challenges \xc2\xb7 Ability to work independently \xc2\xb7 Ability to meet deadlines \xc2\xb7 Ability to work on several projects simultaneously \xc2\xb7 Ability to prioritize work load \xc2\xb7 Demonstrated ability to respond to drug information questions, perform literature searches and evaluate published research \xc2\xb7 Proven ability to attend work regularly \xc2\xb7 Demonstrated proficiency as a hospital pharmacist based on OCP, CSHP and NAPRA standards Salary Range: $48.21 - $64.50 per hour How to apply: Please email HR@psfdh.on.ca and quote job number in subject line.
