JOB SUMMARY:
The Pharmacist practices patient-centered care as part of an interprofessional primary care team to achieve optimal drug therapy outcomes for enrolled patients. They provide comprehensive pharmaceutical care by optimizing medication therapy, resolving actual and potential medication-related problems, and serving as a clinical resource for the care team.
The Pharmacist supports clients with chronic health conditions through comprehensive medication management, collaborating with the primary care team, community pharmacies, and other partners. Responsibilities include conducting patient assessments, managing chronic diseases, and delivering education based on evidence-based guidelines and patient-centered goals.
JOB SPECIFIC RESPONSIBILITIES:
Direct Client Services:
Provide individual client assessments in the management of chronic disease as it pertains to medication therapy.
In consultation with the interdisciplinary team, prepares therapeutic plans to improve medication management for patients with chronic diseases such as diabetes, hypertension, dyslipidemia, heart failure and atrial fibrillation.
Communicate with hospital and community pharmacists and other interdisciplinary team members to ensure smooth transitions of care with respect to medication management.
Provides patient education through individual or group sessions to help in the understanding and management of health issues using pharmacy and therapeutic agents.
Provides ongoing advocacy and support to clients enabling informed decision making.
Conducts patient home visits when appropriate.
Teamwork and Collaboration:
Actively participates in planning, implementation, delivery and evaluation of chronic disease management, health promotion and illness prevention programs and services including medication reconciliation.
Provides primary care team members with current information and guidelines on new drugs, contraindications, therapeutic use of drugs and other information to assist in the provision of quality, evidence-based care to patients.
Works in a manner that incorporates health promotion and recognizes the social determinants of health.
Contributes to the education of health professional students/learners regarding medication safety and reconciliation.
Administration:
Develops/revises policies and procedures related to medication use.
Records and documents care provided within the patients' electronic medical record.
Communication of product alerts, warnings, advisories.
QUALIFICATIONS:
Bachelor of Science in Pharmacy from a recognized university.
Registered and in good standing, Part A, Ontario College of Pharmacists.
Community or hospital pharmacy residency completion or equivalent clinical experience.
Additional training or credentialing in pharmacotherapy (eg. Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma Educator, etc.) considered an asset.
Additional training such as the Structured Practical Experience Program, Teaching Associate Educational Program (University of Toronto) and in Evidence-based medicine principles considered an asset.
Experience with electronic medical records
Experience in program development, implementation and evaluation
Understanding of community resources.
Willingness to update knowledge and skills through various courses, workshops, and conferences.
