About the Opportunity
Reporting to the Clinical Pharmacy Manager and in close collaboration with the Clinical Lead for the Emerging and Re-emerging Infections Network (ERIN), the Pharmacist works as a member of the interprofessional team and participates in the provision of safe, competent, person and family centered ethical care, ensuring the goals and needs of the person are prioritized and individualized. The Pharmacist as part of a virtual consult team is responsible to ensure safe and effective medication treatment and prophylaxis for clients with non-severe & prolonged presentations of emerging and re-emerging infections including supporting care transitions across many treatment settings (i.e., inpatients, ambulatory and LTCF). The Pharmacist, embracing a person-centered approach to care by including clients, families and SDMs in decision-making, is responsible to assess patients for eligibility and clinical appropriateness of therapeutic and prophylactic alternatives, to identify and resolve potential drug-related problems, to collaborate as designated prescribers with Physician partners on therapeutic recommendations and/or adjustments to home medications, and to provide monitoring, follow-up, medication education and counseling on self-care strategies. This role is focused on ensuring equitable access to emerging and re-emerging treatment options across NS and on meaningful communication and collaboration with healthcare partners in acute care, emergency departments, primary care, clinical specialists, infusion clinics & testing centers, continuing care, long term care and community pharmacy, sharing current, ethics-informed, and evidence-based therapeutic information related to emerging and re-emerging infections. The Pharmacist participates in interprofessional team education and continuous professional development activities including research and quality improvement initiatives.
