About the Opportunity
Reporting to the Clinical Pharmacy Manager and in close collaboration with the Clinical Lead for the Emerging and Re-emerging Infections Network (ERIN), the Immunization Lead Pharmacist works as a member of the interprofessional team and participates in the provision of safe, competent, person and family centered ethical care, ensuring the goals and needs of the person are prioritized and individualized. The Pharmacist is responsible for disseminating evidence-based vaccination information to enhance immunization access and best-practices in Central Zone (CZ) Nova Scotia Health facilities as guided by the NS DHW Public Health Division of Communicable Disease, Prevention and Control; and the National Advisory Committee on Immunization. The Pharmacist plans, facilitates and coordinates immunization strategies including targeted inpatient, ambulatory or healthcare worker vaccination clinics, immunization education sessions for interprofessional teams and quality improvement vaccination initiatives as identified by the Emerging and Re-emerging Infections Vaccine Consult Team. The Pharmacist assesses immunization status by completing the best possible vaccination history, completes consents with clients, families or SDM, and administers publicly funded and non-publicly funded (i.e., high risk populations) immunizations for patients in all CZ practice settings. The Pharmacist serves as a local expert for technical support and training in CANImmunize, maintains documentation tools in collaboration with IPP&L (i.e., consent, progress note and order sets), and is an immunization trainer with skills to certify and assess interprofessional immunizer competencies. The Pharmacist works in collaboration with Regulated Pharmacy Technicians to manage CZ vaccine inventory, storage, distribution and preparation. This role requires communication, liaison and collaboration with internal and external partners, most significantly acute care, long term care and Veteran\'s units, IPP&L, IPAC, OHS&W, CANImmunize, Provincial Biodepot, Public Health, and DHW. The Pharmacist participates in interprofessional team education and continuous professional development activities including research and quality improvement initiatives.
