Your Opportunity:
Working in collaboration with physicians specialized in internal and obstetric medicine, nurse practitioners, nurses, and dieticians, the pharmacist will provide integrated clinical pharmacy services to patients referred to the Obstetric Medicine Clinic located at the Lois Hole Hospital for Women, Royal Alexandra Hospital. This ambulatory clinic provides care for patients with pre-existing medical conditions that are to be managed during preconception and pregnancy. The pharmacist working to full scope of practice, assesses patients to develop a medication plan to optimize safe and effective medication use in pregnancy. This involves activities required to resolve drug therapy problems, with appropriate monitoring, follow up and seamless care activities. This is accomplished through actively liaising with members of the health care team and developing a professional relationship with the patient. The pharmacist is responsible for the provision of drug information and medication counselling services and performing administrative responsibilities. The pharmacist may also provide education to other clinic team members or health care provider learners. The pharmacist must be able to triage and prioritize patient care along with other competing demands.

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.
