You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP). Additional Required Qualifications:Active Additional Prescribing Authorization (APA). Newly ACP-licensed pharmacists will have up to 1 year to achieve APA. Preferred Qualifications:Recent (within last year) Inpatient Pharmacist Connect Care practical experience. Successful completion of an accredited hospital pharmacy residency program. Recent (at least 1 year in the last three years) integrated clinical pharmacist patient care experience within an Addiction and Mental Health Program.Please note:All postings close at 23:59 MT of the posting end date indicated.Security Screening:A satisfactory criminal record check and/or Vulnerable Sector Search is required prior to your first day of work.
