The Pharmacist is an integral member of the multidisciplinary team within the Department of Family Medicine, contributing to the delivery of comprehensive, patient and family-centered primary care. Based at the Sunridge Family Medicine Teaching Centre, the Pharmacist provides a broad range of pharmacy services tailored to the diverse needs of patients and their families. Key Responsibilities Clinical Practice & Patient Care: Deliver direct pharmaceutical care including medication therapy management, individualized care planning, and resolution of drug-related problems. Support patients with chronic conditions through safe and effective medication use. Health Education & Resident Teaching: Provide education to patients and families on medication use and chronic disease management. Participate in resident-led education sessions and contribute to the training of medical residents. Interdisciplinary Collaboration: Work closely with all members of the health care team including other Department of Family Medicine Pharmacists, Primary Care Networks, and Community Pharmacies to ensure coordinated and comprehensive care. Leadership & Practice Advancement: Contribute to the advancement of pharmacy practice through consultation, collaboration, and participation in departmental initiatives. Support transformational change and project planning efforts. Quality Improvement & Research: Engage in quality improvement activities and research projects aimed at enhancing pharmaceutical care and patient outcomes. As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.
