Your Opportunity:
The Adult Ambulatory Pharmacist manages medication therapy for outpatients within the Calgary Zone through direct patient engagement. Key activities include the identification and resolution of drug therapy problems, providing patient education surrounding drug therapy and disease state information, prescribing/monitoring/adjusting medication therapies, and providing therapeutic recommendations as part of the multidisciplinary care team. Ambulatory Pharmacists are responsible for the provision of pharmaceutical care according to AHS, clinic, and program standards. In addition to direct patient care, Ambulatory Pharmacists are also responsible for drug information, education, and research activities. They are responsible for maintaining their own professional development and participating in the precepting and mentoring of other staff and students. As well, the Ambulatory Pharmacist will assist with the planning, delivery and implementation of ongoing quality/healthcare improvement initiatives and research programs. This casual position has a primary focus of either HIV care based out of the Sheldon Chumir Health Centre, or adult Home IV Therapy (HPTP) based out of RGH, PLC, and FMC acute care sites (multisite). There are two positions being recruited for, so please indicate your preference in your application details. Note: applicants must be available for orientation a minimum of 3 days/week for the first two months of the position and be available for regular shifts a minimum of 3 days per week after the orientation phase is completed.

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.
