Your Opportunity:Located at both the Sunridge Medical Gallery and Richmond Road Diagnostic and Treatment Centre, the Gestational Diabetes Ambulatory Pharmacist manages medication therapy for outpatients within the Calgary Zone in which they, both initially and on an ongoing basis, perform a comprehensive medication review to identify, resolve and prevent medication-related problems. This is accomplished through collaborating with members of the health care team including the patient and/or their caregivers to formulate a medication treatment plan to optimize patient outcomes. The Pharmacist will serve as the MRHP for initiating medication therapy to GDM patients, and will triage patients for referral to other members of the multidisciplinary healthcare team as appropriate. Duties will include selecting, initiating, modifying, or administering medication therapy, monitoring the patient\'s response to therapy, and documenting all direct patient care activities. Provision of verbal education to patients as well as coordination and integration of medication therapy management services is also an expectation. The Ambulatory Gestational Diabetes Pharmacist is responsible for the provision of pharmaceutical care according to AHS, clinic, and program standards. In addition to direct patient care, the Clinical Pharmacist is also responsible for drug information, education and research activities. They are responsible for maintaining their own professional development and participating in the precepting and mentoring of other staff and students. As well, the Ambulatory Pharmacist will assist with the planning, delivery and implementation of ongoing quality/healthcare improvement initiatives and research programs.Description:As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. Required Qualifications:Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP). Additional Required Qualifications:Licensed in good standing with Alberta College of Pharmacists (ACP). Fluency in the English language (both written and verbal). Computer literacy for drug information and word processing. Excellent communication and interpersonal skills. Advanced Prescribing Authorization (APA) designation. Authorization to administer drugs by injection. Current and ongoing direct patient care experience. Ability to work independently and cooperatively in a team environment. Strong decision-making abilities. Preferred Qualifications:Active CSHP membership. Completion of an Accredited Canadian Hospital Pharmacy Residency. Completion of an advanced degree in pharmacy practice. Certification as a Certified Diabetes Educator (CDE). Recent experience (past 12 months) in diabetes management, endocrinology, women\'s health, and/or internal medicine practice areas. Recent experience (past 12 months) in an Ambulatory practice environment.
