Your Opportunity:

The Ambulatory Pharmacist manages medication therapy for outpatients within Alberta Health Services facilities in which he/she, both initially and on an ongoing basis, performs a comprehensive medication review to identify, resolve and prevent medication-related problems. This is accomplished through collaborating with members of the health care team including the patient and/or their caregivers to formulate a medication treatment plan and work to optimize patient outcomes. Duties will include selecting, initiating, modifying, or administering medication therapy, monitoring the patient\'s response to therapy, and documenting all direct patient care activities. Provision of verbal education to patients as well as coordination and integration of medication therapy management services is also an expectation. The Ambulatory Pharmacist is responsible for the provision of pharmaceutical care according to AHS, hospital and departmental standards. In addition to direct patient care, the Clinical Pharmacist is also responsible for drug information, education and research activities. He/she is responsible for maintaining their own professional development and participating in the precepting and mentoring of other staff and students. As well, the Ambulatory Pharmacist will assist with the planning, delivery and implementation of ongoing quality/healthcare improvement initiatives and research programs. He/she will collaborate with internal and external stakeholders, clinical staff, and patients to develop an evidence-based body of knowledge that will be used to improve service delivery within the specific area of practice.

In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP). Additional Required Qualifications:

Fluency in the English language (both written and verbal). Computer literacy for drug information and word processing. Excellent communication and interpersonal skills. Advanced Prescribing Authorization (APA) designation. Recent direct patient care experience (within past year). Recent experience in Ambulatory Pharmacy Practice (within past year). Ability to work independently and cooperatively in a team environment. Strong decision-making abilities. Preferred Qualifications:

Active membership in the Canadian Society of Hospital Pharmacists (CSHP). Completion of an Accredited Canadian Hospital Pharmacy Residency. Completion of an advanced degree in pharmacy practice (PharmD/Masters). Authorization to administer drugs by injection. Experience working with anticoagulation management, chronic disease management, internal medicine, and/or home parenteral therapy/infectious disease clinics.
