The Pharmacist I will play a key role in providing quality healthcare and will have opportunity to learn and grow within an inter-professional team supporting Alberta Kidney Care North. The ambulatory role will provide care to patients undergoing hemodialysis at the in-center dialysis unit located at the University of Alberta Hospital. The pharmacist will perform comprehensive medication reviews to identify, resolve and prevent medication-related problems. This is accomplished through collaborating with members of the health care team, including the patient and/or their caregivers, to formulate a medication treatment plan and work to optimize patient outcomes. Duties include selecting, initiating, modifying, or administering medication therapy, monitoring the patient\'s response to therapy, and documenting all direct patient care activities. Provision of education to patients as well as coordination and integration of medication therapy management services is also an expectation. Virtual support for dialysis patients in other in-center clinics may also be required. The pharmacist is responsible for providing drug information, education and potential research. Activities for the Alberta Kidney Care North program. Mentorship and student precepting will be an expectation.
