Pharmacist
Job Description
The Pharmacist is a member of the multidisciplinary healthcare team that provides pharmaceutical care to patients at Hamilton Health Sciences. The Pharmacist reviews and assesses medication therapies and works with the patient and the healthcare team directly to optimize medication use and minimize medication-related problems to achieve desired patient outcomes. This work may be performed on individual clinical units, or may be across multiple units to support the medication system needs of the site.
Qualifications
1. BSc. Pharmacy
2. Licensed by the Ontario College of Pharmacists
3. Demonstrated computer skills including pharmacy systems, word processing, spreadsheet and e-mail systems
4. Demonstrated problem solving, teaching, teamwork and evaluation skills
5. Demonstrated commitment to life long learning
6. Ability to work independently, organize and establish priorities
7. Two (2) years experience in a hospital setting or hospital residency preferred
