Carefirst Seniors & Community Services Association
is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.
We are currently seeking a
Pharmacist
to join our GAIN team.

300 Silver Star Blvd., Scarborough and community locations in the GTA
Essential Duties and Responsibilities:
Receive, review, and process referrals (internal and external) for the program.
Perform clinical triage to determine urgency of incoming referrals.
Prepare charts for initial and follow-up visits, including gathering relevant medical history, lab results, diagnostics, and care goals.
Conduct Comprehensive Geriatric Assessments in accordance with standardized protocols.
Represent patient concerns to the team and recommend appropriate discipline involvement.
Perform in-depth medication reviews with deprescribing strategies, adherence solutions, and risk mitigation for geriatric syndromes.
Make dosage adjustments or recommend changes to reduce side effects (e.g., renal dosing).
Facilitate and coordinate referrals to internal and external services.
Liaise with primary care providers and community pharmacies to improve outcomes.
Provide medication-related education to patients, caregivers, and families to enhance safety and adherence.
Act as an in-service resource for GAIN team members (e.g., BPMH gathering, MedRec training).
Assist in policy/procedure development and implementation.
Collect and submit statistical, narrative, and financial data to meet funder and regional reporting requirements.
Qualifications & Job Requirement:
Current Pharmacist licensure (Part A) in good standing with the Ontario College of Pharmacists required
Board Certified Geriatric Pharmacist certification an asset
Minimum 2 years recent clinical experience (preferably geriatric-focused)
Community care and chronic disease management experience preferred
Skills/experience to fulfill major functions of the role including wellness care, assessment, and diagnosis.
Proven ability to coordinate care collaboratively with other interdisciplinary team members as well as to effectively function independently with clients.
Demonstrated ability to relate therapeutically with clients. Includes strong negotiation and conflict resolution skills.
Facilitation skills for education and promotion of health programs.
Demonstrated leadership in the advancement of clinical practice and the achievement of program goals.
Demonstrated use of theory and research/evidence based outcomes in practice.
Access to vehicle, driver's license
A valid vulnerable sector police record check completed within one year
