We are seeking a dedicated and knowledgeable Pharmacist to join our Pharmacy. The ideal candidate will possess a strong background in clinical pharmacy and patient care, with experience in various settings, including acute care and long-term care (LTC) pharmacy. This role involves providing exceptional pharmaceutical care, ensuring patient safety, and collaborating with healthcare professionals to optimize medication therapy.

Responsibilities
Review and interpret prescriptions to ensure accuracy and appropriateness of medications.
Provide patient assessments and counseling on medication usage, potential side effects, and interactions.
Monitor patient progress and adjust medication regimens as necessary to achieve optimal therapeutic outcomes.
Administer immunizations and manage medication administration processes.
Utilize pharmacy software (Kroll), electronic medical records (EMR), and electronic health records (EHR) systems for efficient patient management.
Maintain inventory management systems to ensure adequate stock levels of medications.
Collaborate with healthcare teams in critical care settings to develop individualized treatment plans.
Educate patients on pain management strategies and medication adherence.
Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
Requirements
Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
Active pharmacist license in the state of practice.
Experience in LTC pharmacy or hospital settings preferred; critical care experience is a plus.
Completion of a pharmacy residency or equivalent clinical pharmacy experience is desirable.
Strong knowledge of medical terminology, anatomy, physiology, and pharmacology.
Proficiency in using pharmacy software systems for prescription processing and patient management.
Experience with IV insertion, IV infusion therapy, and compounding medications is advantageous.
Excellent communication skills with a focus on patient service and care.
