Seaway Valley Community Health Centre (SVCHC) is recruiting for a

Pharmacist (28 hours per week, 18-Month Contract)

to cover a leave of absence

.

The Primary Health Care Pharmacist supports clients with chronic health conditions. The pharmacist is an essential member of an interdisciplinary health team aimed at improving client outcomes by optimizing drug therapy (ie: blood pressure, cholesterol, diabetes, pain control, constipation, smoking cessation, etc.). The Pharmacist will conduct client assessments for medication interactions/adverse effects, optimize medication management and provide education focused on key therapeutic areas. The pharmacist will help in navigating drug coverage. The Pharmacist will use electronic medical records.

Salary range between $52.84 to $55.02 per hour
