Responsibilities
Review and interpret physician orders to ensure accuracy and appropriateness of prescribed medications.
Prepare and dispense medications, ensuring compliance with legal, ethical, and professional standards.
Provide patient counseling on medication usage, potential side effects, and interactions.
Collaborate with healthcare teams to develop individualized medication plans for patients.
Monitor patient outcomes and adjust medication therapies as necessary based on clinical assessments.
Maintain accurate records of medications dispensed and patient interactions.
Educate healthcare staff on medication management practices and updates in medical terminology.
Participate in quality improvement initiatives to enhance pharmacy services within the hospital setting.
