Pharmacist
to join our dynamic team and contribute to the health and well-being of our diverse patient population.
The Pharmacist will be responsible for ensuring the safe, accurate, and effective dispensing of medications while offering clinical support and medication counseling to patients. Working in close collaboration with physicians, nurses, and administrative staff, the pharmacist will serve as a key clinical resource within our practice.
Key Responsibilities
Clinical Duties
Accurately review and dispense prescriptions in accordance with legal and professional standards
Provide patient education on medication use, side effects, and adherence
Conduct medication reviews and manage complex drug regimens
Collaborate with physicians on drug therapy decisions and alternatives
Monitor for drug interactions, allergies, and contraindications
Administer vaccinations (if certified)
Participate in chronic disease management (e.g., diabetes, hypertension)
Administrative / Clinic Support
Maintain accurate patient medication records
Ensure compliance with provincial and federal regulations, including OCP guidelines
Communicate with insurance providers and assist in medication coverage navigation
Support inventory management and ordering processes
Utilize
Kroll pharmacy software
for prescription management and documentation
Participate in continuous quality improvement initiatives
Qualifications
Doctor of Pharmacy (PharmD) from an accredited institution
Current licensure in good standing with the Ontario College of Pharmacists (OCP)
Proficient in
Kroll pharmacy software
Strong interpersonal and communication skills
Ability to work independently and collaboratively in a fast-paced environment
Requirements
Proof of pharmacy licensure and education
Professional references
Criminal background check
Must be legally eligible to work in Ontario, Canada
Preferred Assets
Previous experience in a multidisciplinary or primary care clinic setting
Certification to administer injections (vaccines, etc.)
Experience with MedsCheck or other medication review programs
Knowledge of OHIP and private insurance billing
Multilingual abilities are considered an asset
