Unit Summary
Hamilton Health Sciences Pharmacy Services is embarking on an exciting, transformational journey of redefining patient care as we leverage the most advanced technologies to enhance all aspects of delivering highly reliable, safe, and quality care. We are a multi-site department offering a wide range of services within inpatient, ambulatory and retail pharmacy services. We have pharmacy facilities at the Juravinski Hospital and Cancer Centre, Hamilton General, McMaster University Medical Centre, St. Peter's, West Lincoln, and other sites as required including Alternate Health Facilities. The scope of Pharmacy services also includes three retail pharmacies (Cancer Centre Pharmacy, McMaster Drugstore and General Drugstore). We are a tertiary care center offering a wide range of specialty patient services ranging from neonatal, pediatric, adult, geriatric and palliative care and are collaborators for clinical trials and research. Our services are offered with recently renovated state-of-the-art cleanroom facilities for sterile and non-sterile compounding.

Our pharmacy department plays an essential role in ensuring the safe distribution and service of medications to patients which facilitates successful patient treatment outcomes in compliance with legal, regulatory and accreditation standards. The management and distribution of our medications is important in ensuring we can consistently deliver the high-quality, safe care that we are known for. We are growing our pharmacy team and presently seeking a pharmacist to support us.
The pharmacist is a key member of the multidisciplinary healthcare team whose key responsibilities include reviewing and assessing medication orders and collaborating with the patient and healthcare team directly to optimize medication use, identifying, and avoiding medication-related problems to achieve desired patient outcomes. The pharmacist will provide drug information, promote evidence-based/cost effective drug therapy and clinically relevant research and be involved in educational activities for students, families, and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential.
Position Responsibilities:
Responsible for the provision of clinical pharmacy services for the purpose of achieving definite outcomes to improve the patient's quality of life
Review of physician's orders, creation, and maintenance of patient medication profiles, and may include IV admixture, TPN, pediatric and chemotherapy preparation, drug information, drug therapy monitoring and adverse drug reaction reporting depending on the site

Bachelor of Science in Pharmacy required. PharmD is an asset.
Current registration with the Ontario College of Pharmacists required - Part A of Register
Previous hospital experience in unit dose and/or IV admixtures preferred
Hospital residency program or equivalent an asset
Demonstrated ability to work independently and as a member of a team
Demonstrated ability to prioritize a consistent and varied workload
Ability to direct and teach staff.
Demonstrated written and verbal communication skills required, in order to facilitate effective communication with patients, peers, and other health care providers.
Demonstrated ability to identify, prevent and resolve drug related problems.
Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset.
Good understanding of change management; must be able to identify change needs and proactively act on those needs.
Leadership, problem solving, organization skills required.
Effective negotiation and conflict management resolution skills required.
Proficiency in both Official Languages would be an asset.
