This position is responsible for providing personal care and light housekeeping support to seniors in an assisted living environment in Acton, maintaining and enhancing their comfort, safety and physical well-being. As an employee of Links2Care, Personal Support Workers will actively promote, support and represent the Mission, Vision, Values of the agency.
PSW's will work shifts, including weekdays, evenings and weekends. PSW's may be required to work extra shifts on short notice. The work is physically demanding, for example, involving the physical handling and transferring of clients, and requires the use of correct body mechanics. Must be able to lift or move up to 35 lbs.
Shifts are 7:30am - 3:00pm and 3:00pm to 9:30pm.
Role & Responsibilities
Comply with the agency policies, standards, guidelines, and procedures.
Participate effectively in staff and other designated internal meetings.
Maintain confidentiality of all pertinent agency documentation/information.
Provides/prompts personal care for clients which may include, but is not limited to, assistance with standing, moving from one location to another, walking outdoors, dressing, bathing, sponge bathing, showering, skin care, oral care, denture cleaning, hair care, shaving with electric razors, toileting, application of ointments or creams based on client's plan of care.
Provides medication assistance as per the written guidelines of Links2Care.
Performs general household duties for the client which may include, but is not limited to, meal preparation, bed making, cleaning, washing floors, doing laundry, vacuuming, dusting
Effectively communicates with clients to understand the client's preferences and degree of participation in completing personal care and other activities of daily living.
Collaborate and work well in a team environment.
Active participant on the care team, observing any changes/concerns in the client's status and reporting any concerns regarding the health or safety of clients to the Supportive Housing Program Leader, and contributes observations or concerns for use in the annual assessment of client's plan of service.
Collects and maintains client information based on program needs and employer requirements in accordance with Links2Care's confidentiality/privacy policy, PHIPA and PIPEDA legislation.
Maintaining and enhancing our clients' independence, providing safety and comfort and promoting physical well-being.
Accurately maintains client care plans documenting client support activities as per the written guidelines of Links2Care.
Conducts safety checks of clients and advises Program Leader of potential client problems.
Provides emotional and physical support to clients.
Follows guidelines and procedures when dealing with incidents.
Maintains a healthy and safe work environment in accordance with Links2Care's Health and Safety policies and procedures.
Attend in-service learning, training and development opportunities as required.
Maintain records as per agency standards.
Report in writing: all accidents, injuries, and occurrences.
Provide reports as required or requested.
Maintain positive relationships with networks, professional organizations, and other related service providers.
Represent the mission, values, and philosophy of the agency in the community, including networks or meetings as requested.
Assist in the training and mentoring of students, new staff, and/or volunteers.
Set annual performance and professional development goals.
Participate in regular meetings with Program Leader.
Attend relevant conferences, workshops, and other in-service events as approved.
Awareness of agency's Financial and Human Resource policies/procedures.
Account for any program-related funds for which responsibility is granted.
Perform other duties as assigned.
Qualifications:
Personal Support Worker certification.
Standard First Aid/CPR certificate considered an asset.
Completed medication administration training and maintain retraining annually.
Hours of work is to work within a flexible framework.
Experience providing personal support services in a community setting (an asset).
Ability to understand Client Centered Care.
Excellent oral and written communication skills.
Ability to maintain complete and accurate documentation for client records and meet organizational deadlines for administrative processes.
Good judgment and problem-solving skills.
Physically able to perform assigned duties using proper body mechanics.
Ability to maintain strict confidentiality.
Knowledge of and adherence to good health and safety practices and infection control protocols.
Willingness and ability to work weekends, days, evenings as required. Operating hours are subject to change based on client requirements.
Willingness and ability to attend educational workshops as required.
Completion of a satisfactory Criminal Record and Judicial Matters Check (CRJMC) prior to and throughout employment is required.
