We are seeking a compassionate and dedicated Personal Support Worker (PSW) to provide exceptional care and assistance to individuals with diverse needs. Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records
Support medication administration following prescribed instructions while adhering to HIPAA regulations for privacy and confidentiality
Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
Use specialized equipment such as Hoyer lifts or resident's to ensure safe transfers and mobility assistance
Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols
Collaborate with healthcare teams to develop, implement, and review personalized care plans focused on patient-centered goals
Provide emotional support, companionship, and social engagement to enhance overall well-being
Assist with memory care strategies for clients with dementia or Alzheimer's disease, ensuring safety and comfort
Support clients in community settings or assisted living facilities, including home health visits or hospice care when needed
Qualifications
Valid certification in First Aid and CPR from recognized organizations
Experience working in group homes, nursing homes, assisted living facilities, or home & community care environments
Knowledge of patient observation techniques and ability to monitor vital signs accurately
Familiarity with HIPAA regulations ensuring confidentiality of patient information
Prior experience assisting with ADLs, medication administration, and care planning required
Strong skills in caregiving for individuals with developmental disabilities, autism, dementia, or memory impairments
Ability to perform heavy lifting safely using equipment such as Hoyer lifts or resident's when necessary
Excellent communication skills for working effectively with clients, families, and healthcare professionals
Demonstrated reliability in maintaining detailed documentation of patient care activities
