We are seeking a compassionate and energetic Personal Support Worker (PSW) to provide exceptional care and support to individuals in various settings, including home, community, assisted living, and long-term care facilities. In this vital role, you will assist clients with daily activities, monitor their health status, and promote independence while ensuring their safety and well-being. Duties
Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or resident's personal assistive devices
Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately
Support medication administration following HIPAA guidelines to ensure privacy and safety
Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
Conduct patient observation to identify changes in condition or behavior and communicate findings to healthcare teams promptly
Maintain cleanliness of client living spaces by performing light cleaning duties to promote a healthy environment
Implement care plans designed by healthcare professionals, ensuring personalized support aligned with each client's needs
Provide emotional support and companionship to reduce feelings of isolation or anxiety
Assist with mobility and transfers using appropriate techniques and equipment such as Hoyer lifts or resident's personal transfer aids
Support clients in community settings or assisted living environments by helping with errands or social activities as directed by care plans
Experience
Previous experience working in home health, hospice care, nursing homes, or group homes is highly preferred
Demonstrated knowledge of patient observation techniques and vital signs monitoring
Experience assisting with ADLs and providing personal care for individuals with disabilities or age-related conditions such as dementia or Alzheimer's disease
Familiarity with medication administration protocols in compliance with HIPAA regulations
Hands-on experience working with people who have developmental disabilities, autism spectrum disorder, or memory impairments is advantageous
Knowledge of safety procedures for heavy lifting and proper use of mobility aids like Hoyer lifts or transfer belts
Background in long-term care environments or assisted living facilities that emphasizes compassionate caregiving and social work principles
