Duties
Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any concerns to healthcare professionals
Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance
Support residents with meal preparation and feeding, ensuring dietary needs are met while maintaining proper hygiene standards
Utilize specialized equipment such as Hoyer lifts or resident lifts to safely transfer individuals with mobility challenges or disabilities
Maintain detailed documentation of care activities, patient observations, medication administration, and any changes in condition to ensure continuity of care
Engage residents in social activities and provide companionship to promote mental stimulation and emotional well-being
Perform light housekeeping duties including cleaning resident areas to uphold a safe and hygienic environment
Follow all safety protocols including CPR and First Aid procedures to respond effectively during emergencies
Collaborate with healthcare teams to develop and implement personalized care plans tailored to each individual's needs
Support patients with memory care needs such as Alzheimer's or dementia by employing specialized techniques for engagement and safety
Assist with hospice or end-of-life care by providing compassionate support respecting residents' dignity and comfort
Skills
Proven experience in home health, assisted living, nursing homes, or long-term care environments; group home experience is highly valued
Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and HIPAA compliance
Skilled in assisting with ADLs and using assistive devices like Hoyer lifts or resident lifts safely
Familiarity with dementia care, Alzheimer's care, memory care strategies, and working with individuals with developmental disabilities or autism spectrum disorders
Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
Excellent communication skills to effectively collaborate with healthcare professionals, residents, families, and team members
Demonstrated ability to follow detailed care plans while maintaining flexibility for changing needs
Knowledge of infection control practices including cleaning protocols to ensure a sanitary environment
Certifications such as CPR and First Aid are essential; additional training in hospice care or social work is advantageous
