PERSONAL SUPPORT WORKER (PSW), HOSPICE RESIDENCE

REPORTS TO

MANAGER OF RESIDENCE

We are looking for compassionate and dedicated Personal Support Workers (PSW) to join our team in the residence at Hospice Vaughan where we provide hands-on support and meaningful care to residents and their families. Our residence includes a 12-bed hospice facility, providing a peaceful, homelike space for individuals and their families who require end-of-life support.

Full-time, Part-time, and Casual opportunities are available.

JOB PURPOSE/SUMMARY:



The Personal Support Worker (PSW) will be responsible for providing assistance with daily living and end-of-life care to residents and their caregivers. The PSW will work as part of the interprofessional team to implement the plan of care, provide a home-like environment, and offer emotional, social, spiritual, and personal care that reflects the individual needs and preferences of each resident.

PRIMARY DUTIES AND RESPONSIBILITIES:



Care of Residents in the Hospice Residence



Assists with duties related to the admission or discharge of residents as directed by the nursing team.

Carries out the plan of care in a manner that promotes the safety, comfort, dignity, and well-being of each resident.

Maintains a home-like environment while adhering to health, safety, and infection prevention and control standards.

Provides personal care in accordance with resident preferences, including but not limited to bathing, oral hygiene, dressing, perineal care, grooming, toileting, continence care, and skin care.

Assists residents with mobility throughout the hospice using safe transfer and lifting techniques as outlined in their plan of care.

Prepares, serves, and assists with meals, as requested or required by the resident or individual.

Ensures resident rooms are cleaned and tidied daily in accordance with their preferences; disinfects high-touch surfaces as part of routine infection control practices.

Supports infection prevention and control measures in all aspects of care and housekeeping.

Maintains cleanliness and organization of common and storage areas; restocks supplies as needed for the next shift.

Performs other duties as assigned by the Manager of Residence or designated hospice staff.

POSITION REQUIREMENTS



Successful completion of a recognized Personal Support Worker (PSW) program with valid certification.

Completion of Fundamentals of Hospice Palliative Care or Advanced Palliative Practice Skills (APPS) considered an asset.

Minimum of 3 years experience working as a PSW preferred.

Recent experience in providing palliative/end-of-life care is an asset.

Demonstrated client-centred approach to care, with sensitivity to the needs of clients and their families.

Proven ability to work effectively within an interdisciplinary team.

Self-motivated, compassionate, organized, and able to communicate effectively both verbally and in writing.

Demonstrated ability to manage multiple priorities in a fast-paced environment while maintaining professionalism and empathy.

Willingness to participate in mandatory in-service education/training sessions related to skill development, self-care, teamwork, and best practices.

Proficient in computer use (e.g., electronic charting, email communication, TEAMS).

Availability to work a variety of 8- and 10-hour shifts, including evenings, weekends, and holidays.

Physically capable of performing the duties of the position, including standing, walking, climbing stairs, bending, reaching, and regularly lifting or moving up to 25 lbs.

All staff are required to receive the annual influenza (flu) vaccine as part of our commitment to protecting the health of residents, families, and the care team. Exceptions may be considered with appropriate medical documentation.
