PSWs are an integral part of a health care team, and in this role, you will encourage, support, and promote independence for patients in home and community care settings by assisting with activities of daily living under the supervision of our Personal Support Manager.

Overview

We are looking for a Community Support Worker (CSW) to work in our Transitional Care Unit. The work is within a safe community-based transitional environment where patients will be supported in achieving or maintaining optimum functioning while awaiting their next care destination either private retirement home, or long-term care. The program\xe2\x80\x99s aim is to provide short-term transitional care with an estimated length of stay of 45 days.

We are hiring for a 4-month contract.

Full Time Shift work within a 12-hour model - 7am \xe2\x80\x93 7pm.

What The Role Involves
  • Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety
  • Providing cognitive, social and/or emotional support to the patient
  • Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices.
What You Bring

  • Exceptional interpersonal skills
  • Ability to work both independently and as part of a team
  • Availability to work non-traditional hours, such as early morning, evenings, and alternate weekends
  • A valid driver\xe2\x80\x99s license and vehicle
  • Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
  • To protect our patients, we require current vulnerable sector check screening, a clear background check and proof of COVID 19 vaccination*
