Job Summary:

We are seeking compassionate and dedicated Personal Support Workers (PSWs) to join our Continuity Nursing Care to assist our clients in the home setting. In this role, you will provide essential personal care and support services to clients in their homes, enabling them to live independently and with dignity in their community.

Key Responsibilities:

Assist clients with personal hygiene, including bathing, dressing, grooming, and toileting Provide mobility assistance including transfers, walking support, and the use of mobility aids Assist with meal preparation, feeding, and basic household tasks such as light cleaning and laundry Monitor and report changes in client's health status or behavior to supervisors or healthcare professionals Provide companionship and emotional support to clients Follow individualized care plans and maintain accurate documentation Adhere to infection control and safety protocols Maintain client dignity, privacy, and respect at all times Travel between client homes within assigned geographic areas

Qualifications:

Personal Support Worker (PSW) Certificate from a recognized educational institution Current CPR and First Aid certification Valid driver's license and access to a reliable vehicle (if required for the position) Clear vulnerable sector police check Strong communication and interpersonal skills Ability to work independently and manage time effectively Flexibility to work various shifts, including evenings and weekends

Preferred:

Previous experience in home care or community settings Familiarity with electronic charting systems or mobile care applications
