Job Summary



4 shifts per week, mornings 8:30-12:30, evenings 4:30-8:30 (30hrs)

We are seeking a compassionate and dedicated Support Worker to provide high-quality care and assistance to individuals in various care settings. In this role, you will support residents with daily activities, promote independence, and ensure their safety and well-being. Duties



Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote comfort and dignity. Monitor patients' health by taking vital signs such as blood pressure, pulse, temperature, and respiration; report any irregularities to healthcare professionals promptly. Support medication administration following HIPAA guidelines to ensure patient safety and confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist residents with meal consumption when necessary. Maintain clean and safe living environments by performing regular cleaning duties in resident rooms, common areas, and care facilities. Observe residents closely for changes in behavior or health status, especially those with dementia or Alzheimer's disease, providing detailed reports for care plans. Operate mobility aids such as Hoyer lifts or resident lifts to assist with safe transfers while adhering to proper techniques. Follow established care plans designed by healthcare teams to deliver personalized support aligned with each resident's needs. Engage residents in social activities and provide companionship to foster emotional well-being. Document all care activities accurately while respecting patient privacy and HIPAA regulations. Collaborate with nursing staff, social workers, and other healthcare professionals to coordinate comprehensive care delivery. Support residents during emergencies or incidents by following safety protocols and assisting emergency responders if needed.

Qualifications



Prior experience working in assisted living facilities, nursing homes, or home & community care environments is highly preferred. Certification in First Aid and CPR is required; additional certifications such as HIPAA training are advantageous. Demonstrated ability to work effectively with individuals with developmental disabilities, autism, dementia, or memory care needs. Knowledge of medication administration procedures and vital signs monitoring. Experience operating mobility aids like Hoyer lifts or resident lifts safely is desirable. Strong communication skills coupled with patience, empathy, and a positive attitude. Ability to perform heavy lifting when necessary while maintaining proper body mechanics. Familiarity with care plans, social work principles, and long-term care practices enhances your effectiveness in this role.
