Responsibilities



Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; report any concerns to healthcare professionals promptly. Support individuals with memory care needs, including dementia and Alzheimer's disease, by implementing personalized care plans and ensuring a safe environment. Administer medications following prescribed protocols while adhering to HIPAA regulations to maintain confidentiality. Prepare nutritious meals tailored to dietary requirements and assist with feeding when necessary. Maintain cleanliness and hygiene standards by performing routine cleaning tasks within the care environment. Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer individuals with mobility challenges. Document care activities accurately in accordance with facility policies and regulatory requirements. Collaborate with healthcare teams and social workers to develop and update individualized care plans. Engage residents or clients in social activities to promote mental stimulation and emotional well-being. Ensure compliance with safety protocols during patient monitoring and emergency situations; perform CPR or first aid if needed. Assist with transportation for appointments or community outings while ensuring safety at all times. Support hospice or palliative care needs by providing comfort measures aligned with patient wishes.

Skills



Proven experience in group home settings, assisted living facilities, nursing homes, or home & community care environments. Knowledge of patient observation techniques and ability to monitor health status effectively. Familiarity with medication administration procedures and vital signs measurement. Strong understanding of dementia, Alzheimer's, memory care, autism spectrum disorders, and disabilities support strategies. Ability to perform heavy lifting safely using equipment like Hoyer lifts; physically capable of supporting residents' mobility needs. Excellent communication skills for working collaboratively with healthcare teams, residents, families, and other caregivers. Basic cooking skills for meal preparation tailored to individual dietary restrictions. Experience working within HIPAA guidelines to ensure privacy and confidentiality of patient information. Ability to follow detailed care plans meticulously while adapting to individual needs as required. Compassionate attitude combined with patience when supporting individuals facing complex health challenges or behavioral issues.
