Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations
Support medication administration and manage care plans tailored to individual needs
Prepare nutritious meals and assist with meal planning to promote healthy living
Operate equipment such as Hoyer lifts and perform heavy lifting safely to ensure client safety and comfort
Maintain a clean and safe environment through regular cleaning duties and infection control practices
Provide emotional support and companionship to foster a positive environment for clients with dementia, Alzheimer's, autism, or developmental disabilities
Qualifications
Valid certification as a Personal Support Worker or equivalent credential; additional certifications such as CPR, First Aid, or HIPAA compliance are highly valued
Proven experience in home & community care, nursing homes, or long-term care settings
Familiarity with working with individuals who have Alzheimer's disease, dementia, autism or other developmental disabilities
Knowledge of assisted living procedures, hospice care principles, and resident care standards
Ability to observe patient conditions closely and communicate effectively with healthcare teams
Physical stamina for activities involving heavy lifting and prolonged periods of standing or walking
Strong organizational skills to follow care plans accurately and document patient progress diligently
