Duties



Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote independence and comfort. Monitor patients' health by observing vital signs, noting changes in condition, and reporting findings accurately while adhering to HIPAA privacy regulations. Support medication administration following care plans and physician instructions to ensure safe and effective treatment. Prepare nutritious meals tailored to individual dietary needs, including specialized diets for conditions like dementia or allergies. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas, laundry, and sanitation routines. Use assistive devices like Hoyer lifts or other mobility aids to safely transfer residents with mobility challenges or disabilities. Document patient observations, care provided, and any incidents thoroughly in compliance with healthcare standards and facility policies. Collaborate with nursing staff, social workers, and family members to develop and implement personalized care plans focused on holistic well-being.

Qualifications



Current certification as a Personal Support Worker or equivalent qualification required; additional certifications such as CPR, First Aid, or HIPAA training are highly valued. Experience working in group homes, assisted living facilities, nursing homes, or home & community care environments preferred. Demonstrated ability to work with diverse populations including seniors, individuals with developmental disabilities, autism spectrum disorder, Alzheimer's disease, dementia, or hospice needs. Knowledge of patient observation techniques, medication administration procedures, and vital signs monitoring essential for quality care delivery. Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary; physical stamina is important for this role. Strong communication skills with the ability to follow detailed care plans accurately while maintaining confidentiality under HIPAA guidelines.
