We are seeking a compassionate, energetic, and dedicated Personal Support Worker (PSW) to provide exceptional care and support to a couple in their home, transition to a new phase of care. Dementia care experience required. Schedule to begin late-December, possibility of earlier start.
Schedule: Evenings and Mornings, weekend availability required. 12-20 hours per week depending on availability, set schedule.
Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote independence and dignity.
Monitor patients' vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly.
Support medication administration following care plans and HIPAA regulations to ensure safe and accurate delivery of treatments.
Provide companionship and emotional support to clients, including engaging in social activities or recreational programs.
Maintain detailed documentation of patient observations, care provided, and any changes in condition to ensure continuity of care.
Assist with mobility using equipment such as Hoyer lifts or transfer belts; perform heavy lifting when necessary while adhering to safety protocols.
Perform light housekeeping tasks including cleaning client living areas, meal preparation, and ensuring a safe environment aligned with infection control standards.
Qualifications
Valid certification as a Personal Support Worker or equivalent; prior experience in assisted living, home health, nursing homes, or group homes is highly preferred.
Demonstrated knowledge of patient observation techniques, medication administration, and vital signs monitoring.
Experience working with individuals with developmental disabilities, dementia (including Alzheimer's), autism spectrum disorder, or memory care needs.
Certifications in First Aid and CPR are required; additional training in hospice care or HIPAA compliance is advantageous.
Ability to lift heavy objects safely and perform physically demanding tasks while maintaining client safety.
Strong communication skills with the ability to work effectively with diverse populations including seniors and people with disabilities.
Familiarity with care plans, social work principles, and working within HIPAA guidelines ensures confidentiality and quality care.
