General Description
I operate a Family Managed Service (FMS) with funding thru Disability Services. I am the legal guardian responsible for arranging the care of my 67 year old sister who is intellectually challenged. She is easy going person capable of reading and writing similar to a 7 or 8 year old. My sister has a great long term memory for faces and names. She enjoys doing puzzles, watching her favorite shows, going out to a movie, making crafts, shopping and spending time with her family and friends. She is self-sufficient in many ways; loves animals, loves people, sweet treats and specialty drinks from Tim's. My sister takes Access-A-Ride to and from a day program 4 to 5 days a week.
I am looking for a Support Home comprised of an individual, a couple or a family willing to take my sister into their home. The support home will receive $2,045 per month. There is an independently paid respite worker for 4 days per month. Respite is typically scheduled two weekends per month from Friday afternoon at 3:30 PM to Sunday afternoon at 3:30 PM. My family also takes Brenda for holidays and some weekends providing additional time off for the support home.
The FMS administrator (myself), the respite worker and the support home (you) will discuss and coordinated my sister's schedule three to four months in advance allowing everyone input. Obviously if a person is unavailable or has a commitment on a particular weekend they would not be scheduled for that weekend.
My sister will require an adequately sized bedroom and closet for her belongings which include summer and winter clothing, a double size bed, nightstand, dresser, fold up craft table and chair. I will need to look at the bedroom and bathroom arrangement prior as well as prearranged follow-up visits from time to time while she is in your care. You will receive $1,100 for my sister's room and board which is addition to the monthly FMS Support Home payment.
The current FMS contract I have with Disability Services is based on January 1 to December 31 and is renewed each year.
For 2025, I am looking for an individual, a couple or a family to be under contract from April 1st to December 31st with the idea of moving forward into 2026 and beyond.
Support Home Responsibilities and Duties
Duties would include but not limited to scheduling the ride to and from her day program, taking my sister shopping once a week, providing healthy meals and other comforts associated with residential / family life. You will be obliged to deliver a positive home environment for her emotional and physical well-being. My sister has an Individual Support Plan (ISP) and the support home is expected to follow this plan, provide feed-back and be involved with future planning and goal setting.
Ongoing responsibilities also include administering medication, providing light exercise opportunities, monitoring behavior changes, filling out monthly forms and ensuring client safety within and outside the home. The support home is responsible for providing my sister with transportation to her appointments which include monthly massage and chiropractor as well as quarterly dentist appointments etc. For this service you will be compensated with a fuel allowance of $60 per month over and above the other remunerations. The support home is required to notify me of any changes in home setting such plans around moving to a different home, having an individual move to a different bedroom within the existing home or another person moving into the home
Care Giver Qualifications and Skills Required
• Minimum age requirement of 30 years old with minimum of two (2) years related work experience
• A valid Driver's License and reliable vehicle with $2,000,000 legal liability insurance.
• A high level of interactive skills especially working with special needs individuals
• Able to communicate effectively in written and spoken English
• Able to work independently and as part of a team
• Demonstrated ability to follow written and verbal instructions related to client care
• Health Care Aide Certification or related community care experience is an asset
• A clear Criminal Record Check including Vulnerable Sector Check
• First Aid Certification
• Abuse Protocol and Response Certification
