Job Summary



St. Leonard's Place Peel is seeking a

Personal Support Worker

join our health team. Reporting to the Supervisor of Operation you will provide direct care and support to residents in accordance with the organization's philosophy and goals & objectives. This role involves assisting residents with activities of daily living, fostering a safe environment and working towards their rehabilitation and recovery goals.

Key Responsibilities



Client Care



Help residents navigate systems such as healthcare, ensuring they are fully informed and involved in decision-making Assist clients with laundry tasks including washing, drying, and organizing clothing and linens Maintain client's rooms and ensure sanitary conditions Help declutter and organize client living spaces Provide clients with access to their medications in accordance with the agency's medication access policies Establish and maintain positive, professional relationships to foster trust Ensure the client's surroundings and personal effects are clean, tidy and serviceable Cleaning of furniture, cupboards and client's personal belongings to maintain an attractive environment.

Quality Assurance/Health and safety



Adhering to all health and safety polices and procedures. Identifying and reporting any health, safety or environmental concerns to management promptly.

Administration



Maintaining accurate and up-to-date records including daily log of activities, progress incidents and any concerns. Ensuring compliance with regulatory requirements, organizational procedures and polices through proper documentation. Ensure timely reporting of significant changes in clients physical or mental health status.

Stakeholder Collaboration



Collaborating with team members such as care coordinators, counsellors, social workers, healthcare professionals and recreational team. Building and maintaining positive relationships with external agencies including housing authorities, mental health services and social services to ensure comprehensive support for residents. Communicate effectively with the interdisciplinary team to ensure continuity of care and address any concerns regarding client well-being.

Professional Development



Actively engage in team and townhall meetings Participate in mandatory training and ongoing professional development opportunities Access funding for continued education or certifications such as First Aid, CPR, or crisis intervention Lead or support committees and special projects that enhance service delivery and organizational goals

Qualifications



Education and Experience



Personal Support Worker certification from an accredited school Minimum 2 years of experience working with men experiencing homelessness, Mental Health, Substance use and Justice Involvement. Experience in Long Term Care setting Proficiency with Microsoft Office Suite Software

Pay: $22.31-$23.87 per hour
