We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will have a strong commitment to providing high-quality care and support to individuals in various settings, including group homes, assisted living facilities, and private residences. This role requires a blend of personal care, patient observation, and assistance with daily living activities to enhance the quality of life for our clients.
Responsibilities
Provide direct support and assistance to clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal preparation.
Monitor and document patient vital signs and overall health status, ensuring any changes are reported to appropriate healthcare professionals.
Assist with medication administration according to care plans while adhering to HIPAA regulations.
Engage clients in social and recreational activities to promote mental and emotional well-being.
Maintain cleanliness and organization in client living spaces, including light housekeeping tasks.
Provide companionship and emotional support to clients, fostering a positive environment.
Implement care plans tailored to individual needs, including those for clients with Alzheimer's or dementia.
Utilize equipment such as Hoyer lifts for safe patient transfers when necessary.
Collaborate effectively with other healthcare professionals, family members, and social workers to ensure comprehensive care.
Qualifications
Previous experience in home health, hospice care, or long-term care settings is preferred.
Familiarity with working with individuals with developmental disabilities or autism is a plus.
Certification in First Aid and CPR is highly desirable.
Strong skills in patient observation, monitoring behaviors, and recognizing signs of distress or changes in condition.
Ability to perform heavy lifting as required for client mobility assistance.
Excellent communication skills and the ability to work collaboratively within a team environment.
A compassionate approach towards caregiving with a focus on enhancing the quality of life for clients.
