To help fulfill our mission, LOFT is hiring a Community Support Worker.

JOB SUMMARY:

The Personal Support Worker will provide assistance and support on a full-time basis to clients who are living with mental health and addiction issues and who have also been involved with the justice system, living in supportive housing. This includes assisting with personal care and homemaking, including meal preparation, unit cleaning. The Personal Support Worker is responsible for assisting with the social/recreational program for the residents, accompanying the residents to medical appointments and working with community services associated with the personal support program or with partners, family members and friends. This person will also attend meetings in collaboration with the Centre of Addiction and Mental Health (CAMH) team. This person will also be required to monitor the activities of the residents, assess potential crisis situations, provide basic maintenance and cleaning of common areas, preparing/serving meals, supervise the property and keep an up-to-date inventory of supplies. In addition, the staff member will maintain the established routines of the program including all security, fire, infection control, and general health and safety practices.

WHAT YOU WILL DO

  • The position supports the physical health and dignity of the residents, thereby improving their quality of life and enabling them to stay within the program as long as possible, by:
  • Assisting with residents\xe2\x80\x99 daily living activities, such as: dressing/undressing, bathing (especially lifting in/out), toileting (transfers from bed or wheelchairs, incontinence, changing diapers, etc.) Or other personal hygiene (grooming, shaving, teeth, nails, back rubs, skin care, etc.), in accordance with their plan of care or as required; providing housekeeping assistance with their laundry, ironing clothing, cleaning/tidying their rooms, making beds, changing/washing sheets, assisting with meal preparation, serving, feeding and cutting of food; shopping as needed; building a rapport with each resident to meet the daily requirements of their housing environment and promote a healthy lifestyle in consultation with the Program Director;
  • Providing other related assistance as required.
  • Provide daily medication assistance.
  • Ensuring that the personal support supplies/equipment (e.g. walkers, bedpans, gloves, wheelchairs) are available and ready for use; recommending the purchase of supplies for residents and program;
  • Participating in the administration of the program by completing and maintaining the required records such as resident files, activity sheets, check lists, medication sheets, etc. In accordance with established procedures;
  • Communicating with other care providers, hospitals, partners, families and friends as directed by the Program Director; providing socialization and recreation through conversation, card playing or other activities, encouraging interest in hobbies or other pursuits; participating in group activities which promote the wellbeing of the residents and the smooth operation of the house;
  • Supporting the strategies or plans for maintaining normalised daily activities by providing reminders for medications and occasionally accompanying clients on their medical, psychiatric or other appointments; teaching the residents in the activities required for daily living in keeping with PSR principles; maintaining a general awareness of the condition and whereabouts of residents in the house, identifying issues, communicating or noting incidents for the Program Director as required;
WHAT YOU BRING

Must Haves
  • Completion of a post-secondary education such as Personal Support Worker Certificate, Health Care Aide Certificate, Registered Practical Nurse Certificate;
  • A minimum of 3 years of experience in providing personal support to vulnerable adults with special needs including addictions, cognitive impairments, justice involvement and/or mental health issues;
  • Must have current Food Handling Certification;
  • Experience assisting with medications
  • In depth knowledge of the justice system, including the Ontario Review Board
  • Ability to organize activities and work with individuals and groups;
  • Ability to interact with clients in a positive and supportive manner;
  • Physically able to perform demanding body mechanics, (i.e. lifting, transferring, pushing, pulling);
  • First Aid and CPR certification, knowledge of universal precautions;
  • Good oral and written skills in English, fluency in a second language is an asset;
  • Ability to work effectively in a team environment and independently
  • Able to handle stressful situations in a positive manner;
  • Multi-tasking and time management skills related to organizing and prioritizing care.
  • Knowledge of the Residential Tenancies Act and property services
