Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; observe and report changes in condition
Support medication administration following HIPAA guidelines and care plans established by healthcare professionals
Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being
Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and laundry
Use specialized equipment like Hoyer lifts or resident's to ensure safe transfers and prevent injury during mobility assistance
Document care activities accurately in compliance with healthcare regulations and facility policies
Collaborate with healthcare teams, social workers, and family members to develop and implement personalized care plans
Support clients with specialized needs such as dementia, Alzheimer's disease, autism spectrum disorder, or developmental disabilities
Experience
Prior experience working in home & community care, assisted living, nursing homes, or group homes is highly preferred
Knowledge of patient observation techniques and monitoring vital signs is essential
Experience with Alzheimer's care, memory care, hospice care, or working with individuals with disabilities enhances your fit for this role
Familiarity with HIPAA regulations ensures confidentiality and privacy of client information
Hands-on experience with medication administration, care planning, and patient monitoring is advantageous
Ability to perform heavy lifting safely when assisting clients with mobility or transfers using equipment like Hoyer lifts
Previous roles involving social work or long-term caregiving demonstrate your commitment to compassionate support
