Position Summary:

To provide personal care to Long-Term Care residents according to each resident's individualized plan of care.

Assets and Qualifications:

PSW certificate required Must be comfortable with electronic charting and communication Experience working with seniors with complex, age-related diseases and/or mental health concerns an asset Team-work attitude and approach to work Self-motivated with excellent time management skills Current Police Record Check with Vulnerable Sector component required Current TB skin test and COVID-19 vaccination are required

Responsibilities:

Implement the plan of care for assigned residents. Assist with personal care including bathing, dressing and undressing, toileting, general grooming, and ambulation. Observe and report to the Charge Nurse any changes in resident's condition as they occur and document resident changes. Assist in providing Restorative Care nursing therapy according to resident care plan and restorative care flow sheet to promote optimal quality of life. Support the overall Program/Activity process as directed by program staff to promote optimal quality of life. Follow the PSW responsibilities as outlined in the Fire Safety Plan and Emergency Plan. Participate in resident care conferences as required. Attend in-service training and self-development courses as required.
