Peel Cheshire Homes Brampton's Outreach Services provide consumers who live in their own home with attendant care services. Services are self-directed by the consumer and assist them to live their life to the fullest in the community. The Outreach Program provides services between the hours of 7:00 a.m. to 11:00 p.m., 7 days per week.

The Outreach Program is seeking a part- time employee who is able to work independently, is self-motivated, a good problem solver. The position may also require the employee to work statutory holidays

Job Summary



The Outreach Worker provides support services to the consumers in the activities of daily living (ADL). The Outreach Worker enhances the quality of life of the consumers by providing physical support and assistance in directing self-care. In addition, the Outreach Worker will provide an environment that will encourage the Consumer development to their fullest potential in all areas of life.

Accountability



The Outreach Worker reports to and is under the general supervision of the Workplace Resource Manager who is ultimately responsible to the Executive Director.

Requirements:



PSW certification Post-Secondary Education preferred and similar experience working in the community considered an asset. Hold a clean and valid Driver's License and own a reliable insured vehicle (proof of insurance will need to be submitted) Must provide a clear vulnerable sector criminal check Must hold a valid CPR and First Aid Certification Exceptional understanding of the needs of people with physical and intellectual disabilities Ability to organize, co-ordinate, multi-task and work in stressful situations Show initiative, sound judgment, professionalism, flexibility, positive attitude and pleasing personality Attention to detail, exceptional problem-solving skills and have the ability to work both independently and as a team member Demonstrated ability to communicate effectively with consumers and their family

Major Areas of Responsibility



Consumer Care



Making certain that care is provided to each Consumer in a flexible and individualized manner, respecting the Consumer's choices for activities of daily living (for example: showering or bathing, time of meals, etc.) Performing personal care, which may include bathing/showering, grooming, hygiene, mouth care, dressing, undressing, positioning, lifting/transferring, bowel care and toileting Planning and preparing meals as required and assisting with meals and observing safe swallowing of food Being familiar with the various equipment used for Consumer care Homemaking duties including housekeeping, laundry and shopping Respecting the privacy of the Consumer, maintaining confidentiality and respecting personal issues and lifestyles of consumers, families and co-workers Sharing important information with consumers, their families and PCH to maintain an effective and efficient communication system. This includes PCH established reporting and recording procedures Developing job related knowledge and skills by making use of training and learning opportunities and participating in all mandatory training. Maintaining a warm and friendly work atmosphere by dressing and speaking in a professional manner Promoting teamwork among all staff, consumers and management Maintaining familiarity and compliance of all PCH policies and procedures

Health and Safety



Working within the Occupational Health and Safety Act and reporting any known health and safety hazards or violations to the Workplace Resource Manager Practicing universal precautions and using protective devices/clothing required by PCH (for example: wearing gloves, gowns or masks) Reporting any unsafe working conditions, equipment malfunctions and repair needs Adhering to the smoking guidelines of PCH

General Duties



Arriving and leaving work at proper times, reporting appropriately with respect to absences and maintaining an acceptable level of absenteeism Articulating ideas and opinions in a clear and tactful manner. Listening and respecting others point of view, being sensitive to others and responding at a professional level. Being able to see what needs to be done over and above assigned tasks and taking appropriate action Showing confidence in oneself and others, being involved and interested in working and representing PCH positively to the public

Transportation Requirements



Own or access a reliable vehicle for to travel to consumers' homes copy of driver's license to be submitted yearly MTO vehicle driver's abstract to be submitted yearly; employees will be reimbursed for this cost On a yearly basis, proof of Insurance showing that you have a minimum of $ 2,000,000 in liability insurance.
