JOB SUMMARY

Ensure services and supports are client driven in accordance with the identified care plan, goals and objectives, and policies and procedures consistent with the Assisted Living Services Policy, 2023 (ALS Policy) and Senior Services.


Provide to clients living at Deer Park Suites; assisted living services in a manner that promotes wellness, and fosters independence, respect, dignity, and inclusion. Perform required services identified in individual care plans that fall under the ALS Policy encouraging client involvement and degrees of assistance in the areas of: personal support services (personal hygiene activities and routine personal activities of living) homemaking services (laundry/housekeeping), security checks and reassurance services, medication prompts; meal/nourishment assistance life enrichment activities to address psycho/social, health, wellness, and/or community engagement needs. Complete Pharmacy medication training, focusing on medication prompting and client-specific training. PSW- assistance with eye/ear drops, support/assistance with blood pressure checks, and ostomy colostomy care. Collaborate with community health partner providers to follow the integrated care plan as outlined for the client.


Document and report observations of any changes in the clients social, physical, or mental condition, as well as any updates to the care plan resulting from input from the client or SDM. Complete documentation as per policy/established practice guidelines.

QUALIFICATIONS

EDUCATION
---------

Grade 12 diploma or equivalent Completion of an approved Community College - Personal Support Worker (PSW) Certificate Course Additional courses in enhanced PSW / gerontology an asset Unregulated Care Provider Medication training an asset
KNOWLEDGE/EXPERIENCE
--------------------

Minimum of 3 months of current experience in a Supportive Housing/Assisted Living Program, Adult Day Program, or related experience in delivering personal support services to seniors in a personal home setting



SKILLS
------

Must have an excellent rapport with people and demonstrated working ability to understand the unique needs of the elderly Must be willing to obtain the PSW enhanced skills training including Pharmacy medication training. Demonstrated commitment to the VALUES of service, honesty, choice, partnership and respect for clients of Supportive Housing Demonstrated flexibility and resourcefulness in achieving client independence and quality of life outcomes
SPECIAL CONDITIONS
------------------

In accordance with the Corporate Policy and applicable Legislation/Regulation(s), the position requires the incumbent to undergo a Police Record Check and submit a satisfactory Vulnerable Sector Check, as well as complete any subsequent declaration or check as required by legislation/Act, regulation, and/or policy Compliance with any requirements, including immunization and screening measures, of the infection prevention and control program as required by applicable legislation/Act, regulation or policy; some measures are only recommendations (not requirements) + Staff are required to be screened for Tuberculosis in accordance with the applicable legislation/Act
Must provide own transportation and be able to report to alternate locations as identified Must have a good attendance record Must be able to work shift work, including days, afternoons, evenings, weekends and holidays Must be physically and mentally capable of performing the duties of the position.

Currently scheduled 24 hours night shifts, may be required to work days, evenings and nights including weekends. Must be willing and able to work up to 24 hrs per week and declare availability of 32hrs per week.
