The Ontario Shores Centre for Mental Health Sciences is currently seeking a Social Worker, reporting to the Manager of ACTT (Durham site.) The successful candidate will be a Social Worker with sound knowledge of clinical issues in the area of mental health and addiction.
Responsibilities include: participating as a mental health professional on a multidisciplinary team providing assertive community treatment to clients with severe and persistent mental illness; providing client centred treatment rehabilitation and support to referred clients; reviewing referrals in consultation with the team to determine eligibility for the program; developing treatment/ rehabilitation goals and plans in collaboration with clients, their families, their support network and the multi-disciplinary team; providing clinical case management services (i.e. psychosocial interventions, environmental adaptations, delivering medication, monitoring medication compliance, assist and support clients in vivo with activities of daily living, harm reduction related to substance use, vocational rehabilitation, social relationship and use of leisure time); assessing for and recognizing signs of deterioration and institute actions to prevent further hospitalization; provide emergency crisis intervention services to help the client and/or others (i.e. family members, home operators, emergency room physicians) deal with emotional or behavioral crises involving other clinicians as appropriate.
Requirements:
An MSW is required.
The successful candidate must be registered with the Ontario College of Social Workers and Social Service Workers in good standing.
Current CPR HCP is required.
Demonstrated knowledge of Psychosocial Rehabilitation and Recovery.
3-5 years psychiatric experience in a mental health facility or service is desirable.
Demonstrated knowledge of the OCSWSSW Standards of Practice and commitment to maintaining reflective practice standards as described by the College.
Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
An in-depth knowledge and experience of the challenges involved in community reintegration of longer-stay psychiatric inpatients.
Commitment to client transition toward more independence.
Ability to work effectively as part of a multidisciplinary team.
Demonstrated advanced knowledge of the theory and practice of clinical social/mental health work.
Thorough knowledge of social work treatment methods applied in mental health settings/community.
Demonstrated competence in individual, group and family therapy and crisis intervention skills.
Demonstrated knowledge of concurrent disorders.
Experience/knowledge of the Forensic system an asset.
The candidate will have demonstrated excellent interpersonal and effective oral/written communication skills.
The ability to work with clients of diverse ethno racial and cultural backgrounds and proficiency in a second language would be considered an asset.
Ability to perform the physical demands of the position.
Variable hours may be required due to patient demand.
