As we continue to grow, we are looking to add an experienced and detail-oriented Intake Coordinator to our team.

Position Overview:

As an Intake Coordinator, you will be the first point of contact for clients seeking services. You will be responsible for gathering necessary information, assessing client needs, and facilitating the intake process to ensure a smooth and timely start to services. This role requires strong organizational skills, a client-focused attitude, and the ability to handle sensitive information with confidentiality.

Key Responsibilities:

Serve as the first point of contact for new clients via phone, email, and in-person communication. Conduct intake assessments by gathering client information, including changes to personal information and medical history. Maintain and update client records in the database. Communicate effectively with clients, service providers, and other team members. Ensure compliance with all relevant privacy and confidentiality regulations Provide clients with necessary forms and documentation for intake and service enrollment. Answer client questions regarding the intake process, services, and eligibility. Follow up with clients to ensure their needs are being met and services are being provided. Obtain informed consent for any procedures. Ensure patient processing is fast and efficient Keep track of patient status like no show/ late patients
Qualifications:

Strong communication skills, both written and verbal. Ability to manage multiple tasks simultaneously in a fast-paced environment. Excellent organizational and time-management skills. Familiarity with database management and electronic records. Ability to maintain confidentiality and handle sensitive information.
