The Patient Classification Coordinator is a key support responsible for Osler's Rehab Program's implementation of and compliance with the Ministry of Health mandated assessment tools including the National Rehabilitation System (NRS) and Complex Continuing Care Program Continuing Care Reporting System (CCRS).
Accountabilities:
Send out daily reminders to staff to complete the assessments that are due/overdue for the week
Audit assessments to ensure data quality
Finalize and sign off on the NRS/CCRS
Educate staff regarding the assessment tools, including updates
Collaborate with educators and resource nurses to schedule staff training and troubleshoot data quality issues and documentation
Attend huddles when required to address data quality and coding issues
Liaise with and submit data to Canadian Institute for Health Information (CIHI) on a quarterly basis
Data analysis for benchmarking and scorecard support
Make updates and data element changes in the mandated assessments where changes are required in Meditech with the support of the Information Services team
Correct and resubmit all corrections to CIHI
Submit NRS/CCRS assessments to the integrated decision support portal after all assessments have been submitted and corrected at CIHI
Ensure all mandated assessments are completed and submitted to CIHI and correct any submission file errors that may impede assessments from being accepted into the CIHI data base
Ensure all assessments have the correct account number and correct any account number discrepancies
Provide reports for data analysis, benchmarking, corporate scorecard support, length of stay, etc.
Provide NRS/CCRS data for finance
Folder set up for Finance, decision and support program managers and physicians to view
Attend CIHI webinars/teleconferences on a regular basis for NRS/CCRS to ensure any new data elements or changes are communicated to the team members either via e-mail, huddles, in services, on the unit, etc.
Attend the Ontario Resident Assessment Instrument Leadership Group which is a NRS/CCRS Coordinator Ontario/Canada networking group
Qualifications
Undergraduate degree in a relevant health care field; must be a regulated health professional
Two to five years clinical experience, preferable in either rehab or complex continuing care
Required to attend workshops at CIHI for mandated NRS/CCRS and commitment to attend workshops upon employment
Required ongoing education as provided by CIHI to maintain competencies
Excellent organizational skills
Excellent communication skills
Must have specific knowledge of the mandated assessment tools either prior to or upon accepting the role
Knowledge of Meditech software an asset
Must have broad computer skills
Ability to communicate with all team members in the organization
Ability to analyze data and be able to interpret the information
Ability to present the information to large groups

May be required to travel between and work at all Osler sites

Hours: Currently Days, 8 hours, including weekends and statutory holidays (subject to change in accordance with operational requirements)

Minimum: $40.78
Maximum: $50.97
