Patient Care Manager, Ambulatory Urology/general Surgery

Job Description

Sinai Health is looking for an adept and trusted nursing leader to join our team as the Patient Care Manager (PCM) for our ambulatory Urology/General Surgery Clinics. This role reports to the Senior Clinical Program Director for Surgical Services.


As a member of the leadership team, the PCM is responsible for the delivery of safe, high quality patient care, fiscal management, human resource management, staff development and advancing professional practice. As a dynamic leader, you are committed to a philosophy of patient and family centered care and inter-professional collaborative practice. You have the capacity to deliver a high level of patient care and achieve departmental outcomes that are aligned with the Centre of Excellence and the Hospital's strategic objectives. You do so by establishing productive relationships with patients, families, the inter-professional team, and organizational and community partners. Mount Sinai Hospital features a number of ambulatory clinics that support our surgical program, with this role entailing Urology and General Surgery.





In this role you will:




Maintain standards of nursing practice and management of patient care by creating an environment conducive to staff development and excellence in nursing practice Interpret and foster the philosophy, goals and standards of the Department of Nursing Establish unit goals and objectives that are consistent with the delivery of safe, high quality patient care Take a lead role in creating a culture of safety, risk mitigation, quality, and service at the unit level and across the portfolio, to improve practices and service delivery Participate in the programmatic development of the clinical area Manage finances at the unit level through effective budgetary planning, monitoring, controlling and operations Facilitate and provide leadership for resource utilization at the unit and programmatic level Provide leadership in fostering and promoting a collaborative practice environment that is based on mutual respect among all team members Lead, monitor and evaluate continuous quality improvement and program development initiatives that support departmental goals aligned with the Hospital's strategic direction Develop, lead and manage program improvement initiatives based on quality data monitoring and utilize appropriate quality improvement processes Encourage staff participation in quality improvement activities for the unit and preparing quality improvement reports Support the Hospital's strong academic mandate for ongoing staff development and training of students from all healthcare disciplines Recruit, develop and manage staff; complete regular employee performance appraisals and support the development of learning plans Be accountable for the quality of care/service and patient safety at the unit level Promote and maintain a safe patient care and work environment by assessing for and managing actual and potential risks. This also includes engaging in safety audits, promoting staff accountability in workplace safety, and ensuring that adequate structures and processes are in place such as staffing, equipment, supplies, physical space, policies and procedures Establish, promote and maintain good public relations through effective communication within the organization Foster effective labour relations through knowledgeable interpretation and implementation of collective agreements Work with other members of the nursing leadership team to support the implementation of evidence based practice and ensuring the dissemination of research findings to inform nursing practice and improve patient outcomes Participate as a member/leader of departmental and corporate committees and teams Communicate effectively using appropriate lines within the organization Responding as PCM on call as needed Participate in the strategic planning process to drive organizational change and optimization of human resources programming and organizational development Perform other duties consistent with the job classification as required Job Requirements

Job Requirements:




Mandatory


Currently registered and in good standing with the College of Nurses of Ontario (CNO) Master's Degree in Nursing + Bachelor's Degree in Nursing with a Master's Degree in a related discipline may be considered
5 years of recent nursing management experience + Candidates with significant experience in formal nursing leadership roles may be considered
Eligible for cross-appointment to an academic institution

Skills and Knowledge


Proficiency in data interpretation and analysis as a means to support strategic decision making Proven experience successfully leading and facilitating change with multi-disciplinary teams Respected and progressive leader with the ability to inspire, motivate and develop a high performance team Significant experience in quality improvement; success leading, monitoring and evaluating process/systems to identify and achieve efficiencies Proficient in using Microsoft Office Suite; advanced understanding of the value of information technology and experience in the implementation and integration of strategies for systems and technologies Excellent decision making, problem recognition, problem solving, communication, negotiation and conflict resolution skills Flexible with the ability to work efficiently in a fast-paced, multitasking and dynamic environment Excellent interpersonal and communication skills; professional, diplomatic and confident with a proven ability to work with individuals across all levels of the organization and external stakeholders Excellent organizational and time management skills with the proven ability to manage competing demands Model collaborative, cross-portfolio engagement to build consensus and create solutions to complex issues Proven experience leading and/or supporting program development initiatives that align departmental goals with organizational strategic direction Program planning and evaluation methodology skills are an asset Demonstrated satisfactory work performance and attendance history
