Salary positing is on the established grid and is pro-rated to Canopy's fiscal year $56,876.88 to $71,967.40 annually.



The Passport program is looking for a Passport Coordinator to join their team. The Passport Coordinator is responsible for facilitating and monitoring funding agreements for approved individuals within the Passport Program. The Passport Coordinator communicates with individuals awaiting services or funds with PassportONE on an ongoing basis while providing information and communication throughout the process. You will be supporting a caseload of approximately 500 individuals and will be working closely with the Team Lead, Passport to ensure that best practices are followed and quality services are delivered. The Passport Coordinator additionally is involved in fostering community development among agencies and families by providing consultation, advice and orientation to the Passport Program.

Under the supervision of Passport Program Manager with direction from the Team Lead, Passport key accountabilities include:



Manages a caseload of Passport clients as assigned by the manager. Communicates with clients and their families if applicable to provide support in using Passport funds to meet client goals. Monitors client budgets and ensures that clients use funds within the Passport guidelines. Ensures that client information in the database is current and accurate. Is involved with fostering community development among agencies and families by providing consultation, advice and orientation to the Passport Program.

Education, Training and Experience:



Post-secondary degree in Social Work or a diploma as a Social Service Worker or equivalent. 3+ years' of developmental service or case management experience. Annual completion of a Criminal Background Check/Declaration, including Vulnerable Sector Screening (VSS). Non-Violent Crisis Intervention. Completion of OHSA Worker Awareness training, Respect in the Workplace training, AODA training and QAM awareness. Valid G-Class driver's license for the Province of Ontario, access to reliable transportation and have valid proof of automobile insurance with driving for business purposes and a minimum liability of two million dollars.
