Responsibilities

Review and interpret physician orders to ensure accuracy and appropriateness of prescribed medications. Prepare and dispense medications, ensuring compliance with legal and regulatory standards. Provide patient counseling on medication usage, potential side effects, and interactions to enhance understanding and adherence. Collaborate with healthcare providers to develop individualized medication plans for patients. Monitor patient outcomes related to medication therapy and make recommendations for adjustments as necessary. Maintain accurate records of medication administration and patient interactions in accordance with established protocols. Stay current with medical terminology and advancements in pharmacotherapy to provide the best possible care.

Requirements

Active pharmacist license in the state of Ontario Strong knowledge of medical terminology and medication administration practices. Excellent communication skills, both verbal and written, with a focus on patient care. Ability to work collaboratively within a multidisciplinary team to achieve optimal patient outcomes. Commitment to continuous professional development and staying updated on industry trends.
