We are currently recruiting for a Part-time Licensed Practical Nurse!






Job Summary:





Reporting to the Director of Nursing, the Licensed Practical Nurse (LPN) regulated health professional, is responsible for providing planned, resident-oriented, goal-directed nursing care and related services in a Trico Living Well (TLW) community. The LPN adheres to all related provincial/local acts, standards, regulatory health authorities, TLW's standards, guidelines, policies, and procedures, and WELL Building Standards. The LPN must practice and abide by the College of Licensed Practical Nurses of Alberta- Standards of Practice & Code of Ethics. LPNs are required to work to the full scope of practice.




Key Responsibilities:




Works as a member of the inter-professional team at TLW community by maintaining effective communication and good interpersonal relationships with medical and allied healthcare members, residents, families, staff, and other departments; Liaises with residents, and family members and acts as a resource for questions and concerns regarding practices or related health care issues; Completes and ensures resident's clinical care assessments, personalized health care plans/care services, and documentation are accurate, up-to-date and changes communicated as required; Assists in maintaining a healthy, clean, and safe environment for residents, visitors and staff; Plans, coordinates, manages, implements, and evaluates the effectiveness of the personalized health care plan/services in collaboration with resident/substitute decision-makers, physicians, allied health professionals, and team members when changes are necessary; Educates and informs about nutrition, self-care, and disease prevention and promotes health and wellness to residents, families, and staff; Supports and encourages each resident to be as independent as possible according to documented cognitive, physical, and functional abilities; Assists with the scheduling of resident's care conferences and actively participates in these conferences; Coordinates the care team's resident care assignments to ensure efficient and timely delivery of health care services to residents; Supervises, teaches, delegates, and directs unregulated care staff (HCA); Works closely with the Director of Nursing to facilitate resident's move-in, move-out, and transfers; Provides direct resident care and medications/treatments administration and documentation in compliance with regulatory requirements and TLW care standards, policies, procedures, processes, and guidelines; Manages medications as prescribed by physicians, and nurse practitioners, observes changes in condition, and reports to the RN Clinical Lead/ Educator, physicians, and Director of Nursing, completes all assessments, forms, and tools as required; Monitors and administers the safekeeping of drugs and narcotics (including accuracy of narcotic counts) according to medications policies, processes, procedures, and guidelines in compliance with all regulatory and CLPNA standards of practice, and TLW standards, policies, processes, and procedures; Initiates and participates in wellness activities provided for residents as described in their wellness programs/services and encourages residents to socialize and participate in planned activities or programs to develop friendships with other residents; Demonstrates awareness of non-verbal communication and a range of listening skills appropriate to the context of different residents, families, visitors, and situations; Observes, documents, and reports on any changes in residents' mental, physical, and functional condition to the RN Clinical Lead/Educator and/or Director of Health & Wellness; including safety concerns, and physical environment; Responds to and manages significant changes in resident's condition and/or medical emergencies and provides emergency first aid; Coordinates emergency response in the absence of Clinical Lead/Educator of the Director of Nursing/Executive Director; Observes, documents, and reports on residents' health care/services utilizing appropriate forms and tools; Escalates medical emergencies to the Clinical Lead/Educator and/or Director of Nursing immediately; Provides cognitive, social, emotional, and psychological support to residents and families; Understands, adheres, and practices the Health Information Act (HIA), Supportive Living Accommodations Standards (SLAS), Accreditation Standards, WELL Building Standards, Freedom of Information and Protection of Privacy (FOIPP), and Protection of Persons in Care Act (PPCA), and Public Health Act (PHA) and all applicable Acts, Legislations, Regulations, and Standards; Notifies the Director of Nursing of any resident unusual occurrences, safety hazards, illness, incidents, and accidents immediately and completes documentation as per TLW Incident Management Policy& Protocol; Collaborates with other personnel to ensure continuity of health care and wellness services; Cooperates with and assists other agencies providing services to residents; Responds to resident, family, and visitor concerns and ensures appropriate action is taken within decision-making authority, and/or concern is reported to the Clinical Lead/Educator, Director of Nursing/Executive Director and/or other departments; Responds to resident call bells and door alarms promptly; Ensures understanding of and compliance with Resident Rights and Responsibilities; Demonstrates a compassion, patience, and a gentle, non-judgmental approach to caregiving; Understands, practices, and follows all Infection Prevention and Control policies and procedures; Knowledgeable of Occupational Health and Safety Regulations and Emergency Fire Codes and follows in performing job duties; Supports the orientation of new LPN staff upon completion of general orientation; Completes general orientation and LPN orientation, reads all relevant policies, participates in mandatory ongoing training, in-services, and education relevant to the position and responsibilities; Ensures own personal grooming is appropriate and clothes are clean, tidy, and according to dress code; Carries out all duties in a respectful and safe manner; Agrees and abides by the content in the Employee Handbook; including vaccinations as required; Performs other tasks as assigned.

Physical Job Requirements:




This position requires the LPN to be able to: Work Lift up to 40 pounds; Able to work in confined spaces; Pushing, and pulling wheeled equipment up to 50 pounds; Use both hands and feet regularly to perform physical assisting duties; Bend, stooping, on a regular basis; Walk and stand on a variety of surfaces for at least 4 hours per day; Climb stairs, operate elevators, lift and move small equipment and furniture; Good physical and mental health.

Required Education, Credentials, and Experience:




Current registration as a Licensed Practical Nurse (LPN) in the province of Alberta with no restrictions; 2 years + experience working with older adults in the continuing health care/services industry, an asset; A genuine interest and desire in caring for people and improving their daily lives by encouraging resident autonomy to preserve dignity and self-esteem; Valid CPR and First Aid certification required; Demonstrated organizational and time management skills; working cooperatively with others in a team environment; Strong interpersonal, communication, and customer focus skills, kind, patient, and compassionate; Basic computer proficiency and experience required; Current (within 6 months) Police Clearance and Vulnerable Sector Screening.
