In this community nursing role, as a Part-Time Night-Shift Registered Nurse (RN) you will have scheduled visits between the hours of 10pm - 8am to care for patients in the Waterloo Region. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What you'll do:
Using appropriate assessment tools/techniques to assess patients' individualized needs
Executing and continuously monitoring care plans
Engaging patients using a collaborative approach by building rapport and trust
Maintaining and updating patient health records related to encounters, interventions and plans
Ensure care is provided with established nursing standards and principles, physicians' orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents / patients
Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the 'big picture' and generate creative solutions
Ability to work alone and with others while prioritizing tasks to handle the most urgent first
What you bring:
Registration with the CNO as a Registered Nurse in good standing
Valid driver's license and access to a registered vehicle is required
Palliative care experience is an asset
Post-graduate education in Palliative care is mandatory
Experience in delivering community care is an asset
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Passion for making a difference in your community
Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanour
