We are seeking

a dedicated,

compassionate and detail-oriented Licensed Practical Nurse (LPN)

to join us part-time to conduct

client health assessments, care, home safety checks, and reassessments

.

Key Responsibilities:

Conduct comprehensive health assessments for new clients

Perform reassessments for existing clients as required
Complete safety evaluations of client home environments
Identify potential health risks and develop mitigation strategies
Collaborate with caregivers, clients, and families to support safe care delivery
Provide documentation, recommendations, and support for care plans
Maintain accurate client health records in compliance with standards
Support in developing personalized care recommendations
Communicate findings clearly with the ComForCare team and stakeholders

Qualifications:

Active

Licensed Practical Nurse (LPN)

license in good standing with CLPNA Minimum

2 years of nursing experience

in home care, long-term care, or hospital settings Strong assessment and critical thinking skills
Ability to work independently and communicate professionally
Valid driver's license and access to reliable transportation preferred


Passion for senior care and person-centered service


Excellent communication skills and attention to detail
