What this role will do:
Work collaboratively with BFL\xe2\x80\x99s Pregnancy Outreach Program as well as with other community agencies and stakeholders to support the vulnerable young parent population and provide access to relevant community resources and services.
Determine client eligibility for program and conduct comprehensive needs/strengths assessment for each client.
Develop individualized wraparound support plan for each client that is strengths-based, culturally appropriate and supports client to achieve their goals and needs as a parent in alignment with program standards.
Maintain accurate records on participation, one to one session, and referrals; assist clients with relevant applications.
Conduct home visits and accompany client to access relevant services as required.
Organize and participate in client case conferencing with partnering organizations and advocate for clients as and when needed.
Maintain up to date knowledge of community resources and liaise with relevant agencies, professionals, and the community in order to provide wraparound support to client.
Must be able to set and maintain boundaries when working with vulnerable clients (e.g., financial, personal, medical, legal, boundaries) be aware of any conflict of interest, maintain client confidentiality, and be able to work under stressful situations.
Promotes respect and maintains confidentiality of all participants.
Maintain necessary client files, program, and continuous quality improvement reports, statistics, and records. Ensure that all required documentation is accurate and complete according to BFL and the funders\xe2\x80\x99 requirements.
Performs other duties as required.
Education, Training and Experience:
Diploma or degree in a related human/social service field
One-year recent related experience, or an equivalent of education, training, and experience.
Experience in working with Youth from Indigenous and Immigrant communities.
Job Skills and Abilities:
Demonstrated ability to engage and work with youth from different cultural backgrounds including immigrants, refugees and those with low literacy and English language skills.
Demonstrated knowledge of Burnaby secondary schools, MCFD and community resources and services for youth
Excellent interpersonal behaviour, communication, and facilitation skills.
Well-developed administrative, good time, organizational and self-management skills, and ability to function independently.
Ability to work effectively with relevant staff and personnel from Burnaby School District and community and government organizations.
Knowledge and experience working with vulnerable youth population.
Additional Information:
May be required to participate in work-related activities outside regular hours of operation.
Reports To: Manager Family Support Services & Moving Ahead Terms of employment: HOURS: 35 hours/week, with some flexibility according to program needs. A competitive benefits package is provided after successfully completing the probationary period. This is a funded position until March 31, 2025, with possibility of extension.
