FT Contract Role - March 31st, 2026 (with possibly of extension)

INTRODUCTION:



De dwa da dehs nye>s Aboriginal?Health Centre focuses on culturally appropriate, wholistic, primary health care that includes Physicians, Nurse Practitioners, Traditional Healing and other primary health services - Mental Health Support as well as community health supports - Advocacy, Outreach, and Health Promotion and Education Services.

The goal of the Outreach Coordinator is to perform a wide variety of administrative duties to support the Mobile Outreach Van's day-to-day operations.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:



Perform administrative duties such as statistics tracking and reporting, scheduling, posting to patient charts, documenting

patient contacts, clinic inventory/supplies, liaising on behalf of providers e.g., pharmacy, Life Labs, Hamilton public health

Build rapport with community members accessing primary care

Visit encampments, shelters, Indigenous partner agencies and other community locations based on need, as part of mobile

outreach team

Possess a valid G license. Must be comfortable driving large Mercedes Sprinter Van.

STATEMENT OF QUALIFICATIONS:



The ideal candidate would possess some of the following qualifications:

Health or Social Service experience is an asset

Ability to multi-task and interact with diverse clients with compassion, sensitivity, and support

Experience in the field of mental health and addictions, and/or homelessness

Knowledge of Harm Reduction principles and practices, SAFE Talk and ASIST Training, including a familiarity with Indigenous ways of being.

Valid G driver's license and reliable personal transportation

Experience with Electronic Medical Record systems

Intermediate Microsoft Office skills

Excellent organizational skills

Strong written and verbal communication skills.

Job Type: Fixed term contract
Contract length: 5 months

Pay: $57,116.80 per year
